I'm guessing there was something hinky with this particular file. And everything in the picture very clearly indicates that plex is converting to change the container. The TV's built in plex app is not fast enough to play the 4k content, the video stream doesn't buffer, but gets very choppy, where the video is frozen but audio continues until the video catches back up.
