This happens mostly If you had pipboy mods (Pad Style) installed and didnt uninstall them correctly. Reinstall that mod and look at the moddescription how to uninstall them. it's possible a mod unequipped it from your character. I can still open up the pip boy and mvoe around in the menus and select stuff like normal but I only have audio cues to go off on as the entire thing is invisible.
